The Young Adult Transition and Learning Disability Team are a team of social workers working alongside children’s social workers, the young adult, Education, Health Professionals, Carers and a wide range to voluntary and independent sector providers to complete Care Act (2014) assessment to identify eligibility and need.
The team commence an adult assessment usually referred through the Transition Operational Group (TOG) but also through direct conversations with the children’s social worker and/or the referrer; working in partnership with the young person and their family/carer where appropriate.
The detail below is some of the services that can be accessed following a Care Act Assessment although the list is not exhaustive:

A direct payment often creates a seamless transition because the young person will often have been already accessing services through this method. Some young people access:

Supported Internships
A Supported Internship is for young people aged 16-24 who have learning difficulties/disabilities or who are on the Autistic spectrum and would not be able to access work without support. To access the programme you must have an Education Health and Care (EHC) Plan.
Young people in Hartlepool can access a Supported Internship Programme via Choices College, a national programme ran by Health Education England at Hartlepool College of Further Education or at Catcote Academy who run a smaller Internship Programme as a progression route for young people.
The Internship programme runs for 1 school year (September to July) and young people are offered up to three placements within a working environment, each lasting 1 academic term (12 weeks) during their internship to allow them to access a variety of different work experience. Throughout the week 4 days are spent on work placements and 1 day in the classroom studying function skills.
When in the work young people are supported by Job Coaches who help build confidence and skills within the work place, as confidence grows the Job Coach will reduce or increase their support dependent upon the young persons needs.
A Supported Internship is not an Apprenticeship and is an internship model and an education programme, there is no annual salary. You will be able to apply for a bursary, if eligible it will mean you will have some money to help with travel and food.
After completion of the Internship, young people are supported to apply for jobs or further study, the programme has a very high success rate of getting young people into employment or meaningful work experience or volunteering.

One Stop Shop
The One Stop Shop offers young people aged 16-20 years (24years with SEND) a wide range of services including:
- Supporting Education, Employment and Training choices and opportunities based on individual academic abilities
- Job search support including curriculum vitae (CV) support
- Housing and independent living support
- Healthy relationship advice, support and sign posting
- Personal and social development opportunities
- Support to access benefit advice, student finance, child care costing and budgeting
The One Stop Shop is in Middleton Grange Shopping Centre next to the Galley Coffee Shop. We are open Monday – Friday, 10am – 5pm.

Hartlepool Activities and Food Programme (HAF)
Hartlepool Holiday Fun – a project run by Hartlepool Borough Council in partnership with different organisations across the town and funded by the Department for Education – which provides activities for children and young people over the main school holidays throughout the year. As well as having a fantastic time they also get a tasty meal too.
Hartlepool Holiday Fun, part of the Holiday, Activities and Food Programme, offers a wide variety of provision to the children and young people of Hartlepool over the key holiday periods of Easter, summer and Christmas. The HAF Programme offers activities and food to children and young people in Hartlepool in receipt of benefits-related to free school meals of who meet other vulnerabilities.
We took the decision locally to make it so all provision is and will remain, free of charge to all children and young people with an active Education, Health and Care Plan or who are in receipt of Disability Living Allowance.
This holiday provision has featured a good mix of specialist SEND providers such as Springwell School and Families First North East to more universal provision such as multi-sports and arts and crafts and offers such as karate and mindfulness. One thing we have been sure to bring into our ethos is that all providers must offer provision that is adaptable to meet at least a certain degree of SEND needs based on their ability to offer the required levels of support.
Any provider unwilling to offer provision to SEND young people will not be approved for funding. We are also looking to work with the likes of 1 Hart 1 Mind 1 Future to offer training to our providers to better support them in offering provision to SEND children and young people.
