0-19 Services – School Nursing, Health Visiting and Parenting Support

0-19 services is the universal offer which refers to school nursing, health visiting and parenting support. This service is commissioned by the Public Health Team within the Local Authority. 

Public health nurses offer universal support regarding the health and development of all children from 5 to 19 years of age. The service provides interventions on a one to one or group basis to increase resilience and reduce risk taking behaviours. 

All practitioners deliver targeted support to families as needed and have statutory duty to be involved with the child in need and any child protection procedures. The service also undertakes reviews and health assessments for the Looked after Children.

At 2-2.5 years of age your health visiting service will offer your child a developmental review. This review will focus on your child’s social, emotional, behavioural and language development focusing on your child strengths.

It will respond to any concerns about physical health, growth, development, hearing or vision and offer guidance to parents/carers on behaviour management and what to do if they become worried about their child.

It will offer information and guidance on promoting language development and will offer advice on nutrition and physical activity for the family and raise awareness of dental care, immunisations, accident prevention, sleep management, toilet training and sources of parenting advice and information and identify progress towards the Ready for School entitlement.

If a health need is identified during a visit with the Health Visiting service an assessment will be carried out to determine the level of support required. Parents will be asked to consent to the sharing of information with other relevant professionals and parents will be directed to the Local Offer for further support. If necessary, they will gain your consent to refer to the Early Years Panel who aim to support pre-school children who may have additional needs. This is a multi-agency panel set up to support the inclusion of young children with SEND in mainstream settings and improve co-ordination and planning of support.

National Child Measurement Programme (NCMP)

Child and Family Healthy Weight Service providing the National Child Measurement Programme (NCMP) programme and offers one to one / group interventions to children, young people and their families regarding healthy weight and lifestyle.

Disability Register

Registering your child on the Disability Register is optional. They are not required to have an EHCP or a diagnosis. By registering on the disability register you could give you and your family the chance to influence and shape services. You can register your child or young person if they are:

  • aged 0 to 18 years;
  • experiencing development delay in one or more areas;
  • is likely to need support into adulthood; and
  • live in Hartlepool.
