Supported Living

Hartlepool has a range of supported living care providers who we work with (with capacity across the town for approx. 70 people), and as part of a care act assessment, we work with the person, their families, the provider and the social work team to best match the needs of the person with the setting and support required. Pathways to Independence provide support and opportunities for children, young people and adults. Further information about the support available can be found here.

Disability Register

Registering your child on the Disability Register is optional. They are not required to have an EHCP or a diagnosis. By registering on the disability register you could give you and your family the chance to influence and shape services. You can register your child or young person if they are:

  • aged 0 to 18 years;
  • experiencing development delay in one or more areas;
  • is likely to need support into adulthood; and
  • live in Hartlepool.
